The Licensing inspector conducted an unannounced complaint inspection in response to online complaints that were received by this licensing office on June, 2 & 16, 2016. Interviews were conducted with staff and a tour of the center was conducted relating to allegations of physical plant issues, child staff interaction and ratios. The preponderance of the evidence gathered during the investigation supports some of the allegations. A violation notice was completed. Description: Based on observation of the facility and grounds the center failed to maintain the areas in a safe condition. Evidence: 1. The Step 2 climbing equipment has a ladder that has a hole on the top rung and the bottom rung is broken. Both places have pinch points and could snag skin and clothes. 2. In the preschool class the rest mats are torn, exposing the inside padding. (repeat violation)

Plan of Correction: This piece of equipment will be removed from the playground. The rest mats will be replaced or mended.
