Discussed regulations pertaining to medication with the Program Director.
Licensing Inspector arrived for an unannounced monitoring inspection on 7/12/16 from 12:30pm - 2:05pm. During the inspection there were 38 children ages 5 to 12 years old in care with 4 staff. A tour of the facility was conducted and children were observed in classrooms activities, and eating lunch. Records were reviewed for five children and five staff. Medication, emergency procedures, and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).
Standard #: 22VAC40-185-330-B Description: Based on observation, it was determined that the facility did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99. Evidence: 1. The resilient surfacing (interlocking rubber mats) located under the play structure on the playground has multiple areas where the mats have separated and exposed the metal locking brackets. 2. Staff #3 (Program Director) stated that this happened recently and they are not using that area of the playground until the problem is corrected.. Plan of Correction: The play structure and the resilient surfacing are scheduled to be removed on 7/20/16. Until ten, the children will not be allowed to access that area of the playground.
