Unannounced monitoring inspection was conducted beginning 11:00 and ending at 12:26. There were 46 children present, ages 8 months - 11 years. Children were observed doing small group activities, listening to a story about a farm, eating lunch and school age children arrived back at the center after a field trip to the movies. Reviewed 3 staff files - see violation notice Reviewed 3 children's files - see violation notice There were no medications at this inspection. Standard #: 22VAC40-185-60-A Description: Based on a review of 3 children's files, it was determined that 1 child's file was not complete. Evidence: Child #1 emergency contacts without complete addresses. One of 2 required contacts. Plan of Correction: The Assistant Director will notify parents today and get the full address.
Standard #: 22VAC40-185-70-A Description: Based on a review of 3 new staff files, it was determined that one staff had a TB test more than 21 days after starting work. Evidence: The file for Staff A had a TB test on July 1 and had her Orientation on 5/16. Plan of Correction: The administrative staff will assure that all newly hired staff will have their TB tests within 21 days after hire.
