Description: Based on record review, 2 of 3 new employee records checked did not contain complete emergency contact information. Evidence: Staff #2 and Staff#3 records did not contain the addresses of their emergency contacts.

Plan of Correction: Will be corrected today. Will have my employees fill out new ones.

Description: Based on record review, 1 of 4 records reviewed did not have a completed central registry check within 30 days of employment at the facility. Evidence: Staff #3's record does not contain documentation of a completed central registry check. The staff member was hired on 5/2/2016. There is documentation that a request for the background check was made on 6/7/2016, the request was returned by the agency due to an incomplete form. The request was resent on 6/28/2016.

Plan of Correction: Will contact state agency to see where in the process the paperwork is.
