Comments:
An unannounced monitoring visit and facility tour was conducted from 2:18 pm-3:15 pm. At the time of inspectors' arrival there were 53 preschool and school aged children in care with 8 staff members and 1 administrator. Children were observed participating in snack, handwashing, outdoor play, transitioning between activities, and nap. A sample of 5 children's records were reviewed. Medications and injury reports were also reviewed. A sample of 4 staff records were reviewed. Areas of noncompliance are noted on the violation report and were discussed during the exit meeting on the day of the inspection with the Director.

Violations:
Standard #: 22VAC40-185-140-B
Description: Based on record review the center did not ensure that each two to five year old child's physical examination was within 12 months prior to attendance. Evidence: 1. The physical in the record for child #1 age 5(enrolled on 6/20/16) was dated 1/23/15. 2. The physical in the record for child #2 age 5(enrolled on 6/20/16) was dated 5/16/15.

Plan of Correction: The parents were notified on the day of the inspection. The parents will provide updated physical exams for their child's record.

Standard #: 22VAC40-185-60-A
Description: Based on record review it was determined that the center did not ensure that each child's record contained all of the required components. Evidence: The record for child #2 (enrolled 6/20/16) did not contain the complete address for one of the two required emergency contacts and the address, place of employment and work phone number for the father.

Plan of Correction: Parents were notified on the day of the inspection. Parent will give center the missing information to be put in t he child's record.

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined that the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean and safe condition. Evidence: 1. In the Early Childhood room #3 there was: -peeling paint/drywall under the TV -buildup/grime on the bookcase, desk, and reading area 2. In the Early Childhood room #1 there was: -peeling paint/drywall on the wall with the TV 3. On the playground there was: -4 tricycles with exposed rust on the handlebars -1 broken plastic shopping cart toy

Plan of Correction: A work order was put in on the the day of the inspection to address the maintenance and cleaning needs. The toy shopping cart was removed on the day of the inspection.
