Consultation provided on sight and sound supervision. Staff being attentive in reminding the 5/6 year old children to tie their shoes. Volunteers need to be in constant sight and sound supervision of staff.
An unannounced monitoring inspection was conducted during the Summer Program. Thirty eight children were initially present with 5 staff. During the inspection a group of 26 children and 3 staff returned to the center from Sky Zone and 31 children and 3 staff returned from the pool. The Licensing Inspector was able to observe snack, the 11 year old children watching a movie, group activities and clean up and transitions to activities. Seven children had medication on site. Five children and four staff files were reviewed. Arrival time: 2:40 p.m. Departure time: 4:45 p.m.
Standard #: 22VAC40-185-340-F Description: Based on observation by the Licensing Inspector, it was determined that two children under the age of 10, where not in actual sight and sound supervision. Evidence: Two, six year old children were allowed to leave their upstairs classroom and proceed down the steps to the main entrance of the facility without supervision. The two children passed by a door in the stairway which had access to the outside as they descended the stairs. Plan of Correction: We will talk with the staff to figure out what happen and why two children were allowed to leave their classroom unsupervised. The plan is to split the 5/6 year old children into smaller groups which should aide in maintaining better sight and sound supervision.
Standard #: 22VAC40-185-500-A Description: Based on observation and interviews it was determined that children are not washing their hands before and after eating snack nor are they using disposable wipes. Evidence: Twenty three children in the 5/6 year old classroom did not wash their hands or use disposable wipes after or before eating their afternoon snack. Interviews revealed that the children's hands are routinely not being washed after eating snacks. Plan of Correction: We will retrain the staff on our snack/handwashing policy and follow up that training with a memo
Standard #: 22VAC40-185-510-A Description: Based on a review of medication for seven children, it was determined that the medications on site for one child did not give the center authorization to administer. Evidence: Albuterol Sulfate Aerosol and Diphenhydramine HCI was on site for child A but there was no authorization giving permission for the center to administer. Plan of Correction: We will return the medication until we obtain the proper authorization.
Standard #: 22VAC40-185-510-G Description: Based on a review of medications for seven children, it was determined that a prescription medication was not maintained in its original labeled container. Evidence: Albuterol Sulfate Aerosol was not in the original container as prescribed by the pharmacist for child A. Plan of Correction: We will return the medication to the parent and make sure that we are not accepting prescription medication unless it comes in the original labeled container.
Standard #: 22VAC40-185-560-F Description: Based on observation and interviews, it was determined that the snack served did not contain age appropriate nutritional requirement. Evidence: The snack provided to the 5/6 year old children and the 11 year old children ( peanut butter crackers or cheese crackers) did not contain at least one ounce of protein as outlined by USDA for children of that age. The protein was less than 1 ounce. Plan of Correction: We will make sure the children's snack meet the USDA nutritional requirement.
