Comments:
An announced inspection was made to the facility on 7/27/16 at 9:30 a. m. and concluded at 10:30 a. m. The purpose of today's inspection was to measure the fellowship hall for possible child care. The program director met with the inspector and we discussed what was needed to modify the center's license. Please submit a diagram of the fellowship hall, a daily schedule for school-age children, and a detailed diagram of room arrangement. An updated Building Evaluation Form will be needed with the correct capacity and age range that you are requesting. Also, if your operating hours are changing, please include that information in your modification request letter. A current fire and health inspection should be included.
