Arrived 12:20pm Departed 1:45pm An unannounced monitoring inspection was conducted today. Two infants were present with one staff. Three children were present in Early Head Start with one staff. Three toddlers were present with one staff member. Four children, ages two, three and four, were present with one staff member. Seven schoolers were present with one staff member. The children were resting, playing outdoors and eating lunch. Two children's files, and one staff file, were reviewed for compliance. Thank you for your cooperation. Standard #: 22VAC40-185-130-A
Description: Based on the review of two children's files, the facility did not require proof of immunization before allowing a child to attend the center. Evidence: 1. There are no immunizations on file for child #2.

Plan of Correction: An up to date immunization record is now on file for Child #2. 8/2/16 Health Manager/Health Department staff will provide staff training on required immunizations.

Standard #: 22VAC40-185-270-A
Description: Based on a tour of the lunchroom, the facility did not maintain all areas in a clean condition. Evidence: 1. The carpet in the lunchroom is badly stained over most of the room.

Plan of Correction: Children will eat in the classrooms until floor is replaced in dining area. Bids are being procured.
