Comments:
Licensing Inspector arrived for an unannounced monitoring inspection on 8/18/16 from 10:40pm - 1:05pm. During the inspection there were 29 children ages ten months to six years old in care with eight staff. A tour of the facility was conducted and children were observed in classrooms activities, and eating lunch. Records were reviewed for five children and three staff. Medication, emergency procedures, and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations:
Standard #: 22VAC40-185-330-B
Description: Based on observation, it was determined that the facility did not ensure that where playground equipment is provided, resilient surfacing shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Evidence: 1. The fall zone under the swings was not sufficient based on the height of the swing beam. The swing beam is seven feet off the ground, which requires a 14 foot fall zone. There is only 6 feet between the swing beam and the end of the fall zone on the side that faces the side walk as grass is growing in the mulch . 2. Staff #1 (Program Director) confirmed that the fall zone was not sufficient based on the height of the swing beam.

Plan of Correction: A work order will be placed to remove the grass back and replace with mulch in order to create a sufficient fall zone.

Standard #: 22VAC40-185-560-G
Description: Based on observation, it was determined that the facility did not ensure that food brought from home was labeled with the child's name and date. Evidence: 1. In the Infant classroom, there was container of Similac infant formula, Gerber Cereal, and a bag of Yogurt Melts in a clear, plastic tub next to the refrigerator that were brought from home that had the child's name written on them, but did not have a date listed on the them. 2. Staff #4 in the Infant classroom confirmed that the date was not on any of the food items listed above that were brought from home.

Plan of Correction: The dates will be written on any food brought on home will be labeled with the child's name and date.
