Comments:
An unannounced monitoring inspection was conducted at the facility August 25, 2016, from approximately 10:15 am - 1:40 pm. Upon entrance there were 127 children and 14 staff. Additional staff were on site for administrative support. The Licensing Inspectors observed children playing on the playground, putting puzzles together, eating snack, transition from Science activity, preparing for lunch and bathroom breaks. The Licensing Inspector reviewed 5 children's records, 2 staff records, 5 medication records and 5 injury reports. The exit interview was conducted on August 25, 2016 with Program Directors.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record reviewed, the licensee did not ensure staff had documentation of tuberculosis screening within the past 2 years. Evidence: During the inspection conducted on August 25, 2016, staff #2's last T.B. test was completed on February 2, 2013, which is more than 2 years old.

Plan of Correction: The staff will be getting a new T.B. test by Sept. 6, 2016.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the licensee did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: During the inspection conducted on August 25, 2016, the licensing inspector observed the following on the School Age Playground area there were cracked sand buckets , cracked sand shovels with sharp edges and a storage box with cracked toy trucks and sand bucket with sharp edges.

Plan of Correction: All of the cracked equipment will be trashed and replaced.

Standard #: 22VAC40-185-330-B
Description: Based on observation and inspection of the playground, the licensee did not to ensure that the required amount of resilient surfacing was under playground equipment with moving parts or climbing apparatus to create a sufficient fall zone. Evidence: 1. During the inspection conducted on August 25, 2016 , there was an inadequate amount of resilient surfacing in the fall zone under and surrounding the preschool playground ground composite #1. The depth of the pea gravel measured between 1" and 3" and was unmeasurable in some areas. 2. There was an inadequate amount of resilient surfacing in some areas of the fall zone for the preschool swings. The depth of the pea gravel measured between 0" to 3" . 3. There was in adequate amount of resilient surfacing in the fall zone under and surrounding the School Age composite playground equipment. The depth of the pea gravel measured in fall zone between 1" and 5".

Plan of Correction: They will have maintenance staff working on the playground and will make sure that the pea gravel is efficient.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the licensee did not ensure requirements are met when children are diapered in a classroom. Evidence: 1-During the inspection conducted on August 25, 2016, in the toddlers classroom there two apple place mats touching the diapering surface. 2-In the toddler's classroom, the foot operated diaper pale lid would not open.

Plan of Correction: The diapering surface will make sure teacher cleans the diapering pad and does not put anything on the diapering surface. Will replace or fix diapering pale by August 29, 2016.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on 1 of 2 records reviewed, the licensee did not ensure updated background check dated with in the past 3 years. Evidence: During the inspection conducted on August 25, 2016, Staff # 2's most recent central registry findings was dated March 27, 2013, and most recent criminal history record check was dated March 21, 2013.

Plan of Correction: The Criminial record check and Central Registry findings was mailed out in August.
