The licensing inspector conducted an unannounced inspection in response to a self-reported concern regarding forbidden action. The self-reported concern was received by the licensing department on August 19, 2016. An inspection was initiated on August 22, 2016 and was completed on August 26, 2016. On August 22, 2016, the inspector was on site from approximately 10:30am until 12:45pm. Interviews with staff were completed, observations were made at the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the self-reported concern is determined to be not valid. Though not a part of the original report, there was a violation found during the inspection regarding children's records. Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the violation notice and returned it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word 'corrected' is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Standard #: 22VAC40-185-130-B Description: Based on review of two children's records, one record did not contain documentation of additional immunizations at least every six months for children under the age of two years. Evidence: During the inspection, the licensing inspector observed the record for Child #2 (enrollment date 09/30/13) contained immunization record dated 08/26/13. This immunization record was the only one in the child's record. The child was approximately seven months old at the time of the enrollment. The immunization record was not updated at least every six months until the child was two years of age. Plan of Correction: Updates have been obtained. We will be sure to get updates every six months until the age of two years.
