Technical Assistance:
1. Reviewed employment record requirements for staff that transfer from one center to another, or those that come from another center to work as needed. 2. Discussed containers used for story toys.

Conducted an unannounced monitoring inspection 11:25am-1:05pm. Observed 53 children + 8 direct-care staff. Ratios and supervision were in compliance. An Acting Director has been covering the center for a period of weeks. Children were observed participating in a soccer instruction program, having indoor free-play, story-time, washing hands, and eating lunch. Infants were napping, being held and fed, and were having playtime on the floor. Lunch served: cheeseburger, roll, mixed vegetables, peaches, and milk. Classrooms were found to be clean, safeguarded appropriately, and sufficiently supplied with toys and equipment for the children. Exterior building maintenance was lacking and violations found, to include the outdoor playground equipment and areas. Medication management was in compliance. Required documents posted. Standard #: 22VAC40-185-70-A
Description: Based on records reviewed, it was determined that 2 of 3 staff records were incomplete or not on-site today. Evidence: 1. Staff #13 - There was no documentation of name, address, age verification, job title, date of employment, emergency contact information, two reference checks, education level and previous work experience needed to qualify for the Lead Teacher position she was working today, health information, and staff development. Staff #13 was observed working alone as a Lead Teacher in the toddler-age classroom today. 2. Staff #15 - There was no documentation of education level and work experience, as required to qualify for the Lead Teacher position she was working today. Staff #15 was observed working as a Lead Teacher in the 3's-4's classroom today.

Plan of Correction: Staff [Name] file now goes with her to and from her center and us. Copy was made for her here. No longer working/helping us. 9/2/16. Teacher is working on providing her education transcripts/diploma work experience resume being attached 9/15/16

Standard #: 22VAC40-185-270-A
Description: Based on observations made, it was determined that areas and equipment of the center was not maintained in a safe and operable condition. Evidence: 1. Concrete sidewalk - There was broken concrete in the outdoor play area, which has resulted in a hole that posed as a potential hazard. The hole in the concrete was approximately 2" wide x 2" length, and the dirt foundation under the concrete has eroded away leaving a hole approximately 5" in depth. [Photos #1 and #2] 2. Playground Climbing Structure - Rusted bolts on the outdoor climbing structure were present, and had been painted with black paint, but evidence of the rusted bolts were visible and no actual repair/replacement have occurred. [Photos #3 and #4] 3. Building Exterior - Extensive peeling paint, splintered and cracked wood, a broken downspout, broken fencing, and sharp metal were present on exterior areas of the center. The broken fencing caused a nail to be exposed, and a 1/4" sharp edges on a broken metal screen were within reach of children. [Photos #5, #6, #7, #8, #9, #10, and #11]

Plan of Correction: Facilities is sending someone out tomorrow to fill the holes in concrete, Friday to re-fill ensure it is done correctly. 9/9/16 Contacted facilities, facilities coming out tomorrow to correct all rusted/spray paint bolts-doing one playground at a time. 9/15/16 Facilities have been working on the whole exterior, building all new roof, boards, and underneath replace 9/07/16

Standard #: 22VAC40-185-550-D
Description: Based on records reviewed and interviews conducted, it was determined that a monthly evacuation drill was not completed. Evidence: There was no documentation of emergency drills completed for May, June, and July 2016. Staff stated that drills were not done during these months.

Plan of Correction: We already did a fire drill for September and shelter in place next week. We will continue to work on all emergency drills.
