An unannounced monitoring inspection was conducted on September 27, 2016. The director was present for the inspection. There were 5 four year old children present with 2 staff. Four of the children were playing with toy trucks and cars with one of the staff, and the fifth child was with the director for a period of time. There were 12 three year old children with 2 staff, with the children playing dress- up with various costumes and clothing. Later all of the children ate their lunch in the cafeteria area. Lunch for the day included a hoagie with lettuce, tomato, and cheese, cooked cabbage, bananas, and milk. School age children were not present. Four children's files were reviewed and 1 staff file. Inspection began: 10:55 Inspection ended: 11:55
Standard #: 22VAC40-185-70-A Description: Based on a review of staff record, the center did not have all of the required documentation on file for a newly hired staff. Evidence: The record for Staff # 1 did not include documentation of an emergency contact, any health problems, 2 references, and the background and experience of the staff. Plan of Correction: The documentation will be on file today.
Standard #: 22VAC40-185-550-F Description: Based on observation, the center did not have 911 or a local number for police, fire and emergency services and the number for poison control posted by the phones. Plan of Correction: The numbers will be posted today.
