Discussed with the Director: Assuring that the School gets you a copy of the Fire Inspection Report; Also discussed the serious nature of the MAT training for a specialized condition
Unannounced monitoring Inspection was conducted beginning at 6:05 and ending at 7:20. By 7:20 there were 29 children present with 4 staff. Children were observed as they arrived, working in Zone activities and learning about Rain Forests in group time. The children were very attentive during all of their activities. Reviewed the Parent Board - complete postings but please assure that the last Violation Notice is posted where it is easily accessible to parents and not behind the Inspection Summary, Reviewed medications for 4 children - Medication documentation is complete Reviewed 4 staff files - see Violation Notice Reviewed 5 children's files - complete If you have any questions please notify Linda Reynolds at 540-272-2663 or email firstname.lastname@example.org
Standard #: 22VAC40-185-510-B Description: Based on a review of staff files and interview with the Director and staff it was determined that there was no one on staff certified to give a certain medication to Child A. Evidence: Child A has a medical condition that requires a medication in case of seizures. No one on staff has the MAT (medication Administration Training) that is necessary to administer that medication. Plan of Correction: The Director states that the staff will be trained on line today for this medication.
Standard #: 22VAC40-185-550-A Description: Based on a review of the Emergency Preparedness Plan, it was determined that the Plan was not complete. Evidence: Name and respinsibilities are not completed throughout the Plan Plan of Correction: The Plan will be completed including staff responsibilities.
Standard #: 22VAC40-191-50-A-2-a Description: Based on a review of staff records, it was determined that one of 4 staff did not have a Child Protective Services Central Registry Check on file. Evidence: Staff #1, whose date of employment was 8/18/16, did not have a CPS form in her file. She is currently working. Plan of Correction: The Director will check with the office to see if it is there. Staff #1 will not work until she has a valid CPS Check,
