Technical Assistance:
Centers operating five or more hours per day shall have a designated rest period for at least one hour but no more than two hours reflected on the posted daily schedule in each preschool classroom.

Comments:
An unannounced monitoring inspection was conducted on October 5, 2016. There were 43 children present with six staff members and a volunteer in three classrooms. The children were observed playing in centers, having storytime, writing in journals, and having lunch. One medication was reviewed during this inspection. Three children's records were reviewed during the inspection.

Violations:
Standard #: 22VAC40-185-60-A
Description: During the inspection conducted on October 5, 2016, the facility did not ensure that each child's record was documented with all required information. Evidence: 1. During the inspection conducted on October 5, 2016, the record for Child #1 did not include the documentation of the following: employment information for both parents, the name of any previous child day care and schools attended by the child; name of any additional programs or schools that the child is concurrently attending and the grade or class level; and documentation of viewing proof of the child's identity. 2. The record for Child #2 did not include documentation of viewing proof of the child's identity. 3. The record for Child #3 did not include documentation of the following: viewing proof of the child's identity and a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately.

Plan of Correction: Birth certificate information will be submitted along with employment information, previous schools and additional schools attended by the parents. The parental consent form will be filled out and the student data verification form.

Standard #: 22VAC40-185-540-D
Description: Based on observation and inspection of the facility, the licensee did not ensure that all required emergency supplies were at the center. Evidence: During the inspection conducted on October 5, 2016, the activated charcoal preparation for the center expired in January 2016.

Plan of Correction: An order has already been placed for new charcoal.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation and inspection of the facility, the licensee did not ensure that the findings of the most recent inspection of the facility were posted. Evidence: During the inspection conducted on October 5, 2016, the facility had the findings of the inspection conducted on 3/24/16 posted but the most recent inspection for the facility was conducted on 6/17/16.

Plan of Correction: The findings from this inspection will be posted.
