Technical Assistance:
Consultation was given on obtaining background searches in a timely manner. It is important that the facility maintain compliance as it has had repeat violations in this area prior to this inspection. Staff without a completed Criminal Record Check may not supervise children alone and must be in sight supervision of another staff member with a completed Criminal Record Check. Both staff are approaching the 30 day time period and the facility must follow through to verify the forms were received by the appropriate search agency to continue to be employed. Contact with the agency must be documented. A MAT certified member must be on-site at all times when children are present with emergency medication. Only one staff currently has MAT certification. The other credential by the American Red Cross for administering Epi-pens is not accepted.

An unannounced monitoring visit was conducted today. Twenty six children were present with four staff and in direct ratios for supervision. Snack served today was crackers, cheese sticks and water. Two emergency medications were on-site with one staff having MAT. No injury forms have been completed as of this date. Drills and emergency supplies are current. Observed transition into the after school program, hand washing, snack and going outside to play on the blacktop. Areas of non-compliance are identified in this report. Description: Based on record review, no documentation is available to verify that staff received orientation training by the end of the first day of assuming job responsibilities. Description: Based on medication review, two on-site medications were missing physician authorization for long term medication. Evidence: Medication for child #1 and child #2 did not have physician authorization which is required when medication is kept on-site longer than 10 days.
