Technical Assistance:
Inspector spoke with director about TB screenings during this inspection.

Comments:
An unannounced monitoring visit and facility tour was conducted from 10:25 am-11:55 am. At the time of inspectors' arrival there were 78 toddler-6 year old children in care with 8 staff members and 2 administrators. Children were observed participating in Montessori activities/instructional time in the classroom, transitioning between activities and hand washing. A sample of 5 children's records and 3 staff records were reviewed. Medications and injury reports were also reviewed. Areas of noncompliance are noted on the violation notice and were discussed with the Director during exit meeting at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review it was determined that the center did not ensure that each staff record contained all of the required components. Evidence: The record for staff #2(hired 10/7/2015) did not contain written documentation of the orientation training received.

Plan of Correction: Director and staff will complete the required orientation documentation.

Standard #: 22VAC40-185-260-A
Description: The center could not provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: The fire inspection available was dated 3/25/15.

Plan of Correction: The director contacted the Fire Marshall's office and requested an updated fire inspection be performed.

Standard #: 22VAC40-185-550-M
Description: Based on record review it was determined that the center did not ensure that the written record of children's serious and minor injuries contained all of the required components. Evidence: Five of the eight injury reports viewed did not contain all of the required components.

Plan of Correction: Director and staff will ensure that all injury reports are completed fully.
