Comments:
An unannounced monitoring inspection was conducted today, October 19, 2016 to check for compliance with Licensing standards. The inspector initiated the inspection at 10:00 am and concluded the inspection at 12:00 pm. Today there were 15 children in attendance with two staff. Upon the inspector's arrival children were playing in centers (play-dough, house keeping/dress-up, and art). Today's art project consisted of coloring a cd with markers and gluing a marble in the center to create a spinning top. Children carved a jack-o'-lantern and estimated how much the pumpkin weighed. Later children had lunch in the elementary school cafeteria. Today's lunch consisted of pulled pork BBQ on a whole grain bun, baked beans, fruit and milk. Children were also observed on the playground where they played on climbing equipment, with balls, and sidewalk chalk. Children were excited to watch trucks pass by the playground and blow their horn. Staff circulated around the playground and initiated group games. Systems were in place to check the playground before children entered for any hazards. All areas of the center were inspected to include the main hallway, cafeteria, bathroom,playground area and classroom. The facility had all required information posted to include: the License, last inspection notice, emergency numbers, menu, evacuation map/information, daily schedule along with various parent information and lesson plans. The classroom had children's work displayed with age and stage appropriate materials. The first aid kit and non-medical supplies were inspected and found complete. The inspector interacted with staff and children in care. Three children's records and two staff records were reviewed. Staff have current CPR, first aid, daily health observation training and MAT. Medications and authorizations were reviewed. The most recent evacuation drill was conducted on 09/16/2016. A shelter-in-place drill was documented on 09/23/2016. A second shelter-in-place drill will need to be completed by the end of the current school year. Fire Inspection: 06/04/2015 (see violation notice) Health Inspection: 09/22/2016 Violations from the previous inspection on April 26, 2016 were checked for corrections. Description: Based on review of records and interview, the facility did not have documentation of the elementary school's annual fire inspection. Evidence: The most recent fire inspection notice for the elementary school was dated 06/04/2015, exceeding the annual requirement for a licensed program.

Plan of Correction: Per the site director: We have requested a copy of the fire inspection report, however the school has not provided us with the information. We will request the report again.
