Comments:
An unannounced monitoring inspection was conducted from 8:10 am-9:15. At the time of entrance, 38 children were in care with 3 staff members present. Children were observed arriving to the program, making french toast and playing in the gym. New rooms # 116 and #117 were measured. A selection of staff trainings, children records and medications were reviewed. The violation was reviewed with the center surpervisor. Description: Based on a review of children records, the center did not maintain a complete record for each child enrolled. Evidence: Child # 1 and #2 do not have a record of their physical and immunization records.

Plan of Correction: SACC staff will request the records from the school office.
