22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was initiated today at 12:00pm. The physical plant, 4 staff files, 4 children's files, annual fire inspection, staff training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected today. There were 26 children under the supervision of 5 staff. During the inspection, children were observed listing to teachers read stories preparing for and eating lunch. The children bring their lunch from home each day they are in care. The interactions between the children and staff were seen to be positive and constructive in nature. There are currently 2 staff certified in MAT (medication administration training). However, there are no children with any medication needs during program hours at this time. Standard #: 22VAC40-185-140-A
Description: Based on file review, 1 out of 4 children's records does not have a physical examination. Evidence 1. Child #4 does not have a physical on file.

Plan of Correction: We will ask the parent to bring this into the center.

Standard #: 22VAC40-185-270-A
Description: Based on observations, there was an area outside the center which was not maintained in a safe manner. Evidence 1. The corner of asphalt in the children's play area closest to the entrance to the center is deteriorating. There are railroad ties which were placed on either side of this corner. The wood is cracked and splintering, exposing the metal pins which are now protruding out from the wood.

Plan of Correction: We will notify the church to have them address this.

Standard #: 22VAC40-185-280-B
Description: Based on observations, there was a hazardous substance in an area accessible to the children. Evidence 1. In the Panda classroom, there was a small Raid ant bait on the floor next to the door that leads outside.

Plan of Correction: This was removed during the inspection.

Standard #: 22VAC40-185-330-B
Description: Based on observations, where playground equipment is provided the resilient surfacing does not meet the minimum safety standards. Evidence 1The tire swing and the exit of the metal slide has less than six inches of mulch underneath and in the fall zones.

Plan of Correction: There is sufficient mulch in the playground area. This will be redistributed under these areas.
