An unannounced monitoring inspection was conducted on October 21, 2016 during the before care portion of the program. There were seven children present at arrival to the facility with two staff members. Throughout the course of the inspection there were a total of 24 children present. The children were observed playing with various table top activities to include Legos, board games, playdoh and slime. Staff were observed using positive age appropriate behavior management intended to redirect children's behaviors. The licensing inspector reviewed the physical plant, supervision, required postings, children's records, medication, emergency evacuation and shelter-in-place documentation, and nutrition. The posted menu indicated that today's snack consisted of goldfish crackers and milk. One medication was reviewed. Three children's records were reviewed during the inspection.

Description: Based on record review, in one of three children's records reviewed, the licensee did not ensure that each child's record was documented with all required information. Evidence: During the inspection conducted on October 21, 2016, the records for Child #1 and Child #2 did not include documentation of the work phone number for one parent.

Plan of Correction: Site Director will update the files.

Standard #: 22VAC40-185-240-D-4
Description: Based on observation and inspection of the facility, the licensee did not ensure that any child for whom emergency medications (such as but not limited to albuterol, glucagon, and epipen) have been prescribed shall always be in the care of a staff member or independent contractor who meets the requirements in subdivision 1 of this subsection (Medication Administration Training-MAT). Evidence: During the inspection conducted on October 21, 2016, there were two staff members present without current documentation of Medication Administration Training. There was a child in the group on the date of the inspection who has been prescribed an emergency medication. The medication was present at the facility and viewed by the licensing inspector.

Plan of Correction: We will make sure that we have a person that is MAT certified at all times.

Standard #: 22VAC40-185-510-E
Description: Based on observation and inspection of the facility, the licensee did not ensure that medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given. Evidence: During the inspection conducted on October 21, 2016, the licensing inspector reviewed an emergency medication that was not labeled with the child's name and the time or times that medication was to be given.

Plan of Correction: I will ask the parent to get a prescription label.

Standard #: 22VAC40-80-120-E-1
Description: Based on observation and inspection of the facility, the licensee did not ensure that the most recently issued license was posted. Evidence: During the inspection conducted on October 21, 2016, the most recently issued license was not posted. The facility had a license posted with the effective dates March 8, 2014-March 7, 2016.

Plan of Correction: Corrected during the inspection.
