Comments:
An unannounced monitoring inspection was conducted on October 18, 2016, between the hours of approximately 10:16 a.m. and 2:05 p.m. Sixty-one (61) children were enrolled and fifty-four (54) children were in the care of ten staff members during the on site inspection. Two additional staff members were present and assisted, as needed. Five children's records and ten staff records were reviewed and/or updated. One child's medications and accompanying authorization forms were reviewed. Safety equipment (first aid kit, flashlight and radio), posted information (license, licensing inspection documentation, emergency evacuation diagram, emergency telephone numbers, and classroom schedules) and available documents (emergency evacuation drill & shelter-in-place records, attendance logs, infant daily records, injury report forms and local agency report) were observed/reviewed. The daily schedules accurately reflected the activities provided. Lunches and snacks were provided by the children's parents. Activities observed on this date included: indoor play (toys and stories); preschool learning/art activity - painting pumpkins with baking soda and vinegar; restroom and handwashing procedures; lunch; naptime; infant care; and arrival of one child. Staff/child interactions were observed to be positive and encouraging of both self-help skills and children's emerging independence. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Substantial compliance with the licensing standards continues to be maintained; the exception to compliance was noted on the accompanying violation notice. Standard #: 22VAC40-185-160-C
Description: Based on review and/or update of ten staff records and discussion with staff #1 on October 18, 2016, facility staff failed to obtain the results of a follow-up tuberculosis screening for one staff member, at least two years from the date of the first initial screening or testing. Evidence: 1. Update of staff #5's record, on this date, revealed that the results of an updated TB screening were not obtained (date of initial screening/testing was 7/10/2014). 2. Staff #1 confirmed that an updated TB screening/test had not been obtained.

Plan of Correction: The results of an updated TB screening/test will be obtained from the staff member and added to the facility record. In the future, results of follow-up TB screenings/tests will be obtained at least every two years from the date of a staff member's first initial screening or test.
