Comments:
An unannounced monitoring inspection was conducted today with the director, who was present with two additional staff and 46 children. An additional child joined the group after finishing being tutored. The children had finished snack ( cinnamon toast crunch and milk) and were transitioning to hand washing. Afterward, the children engaged in homework time. Five staff and six children's files were reviewed. Seven children had emergency medication on site and there was one documented injury/accident report reviewed for the current school year. Fire inspection: 4/21/16 Health inspection: 12/11/15 Arrival time: 4: 10 pm Departure time: 5: 40pm

Standard #: 22VAC40-185-70-A
Description: Based on a review of five staff files, it was determined that one staff's file did not contain all of the required information set forth in this standard. Evidence: There was no documentation on file as to when staff A was hired. Interviews revealed that it was the end of September or early October 2016

Plan of Correction: We will make sure to document the hire date in each staff file.

Standard #: 22VAC40-185-240-A
Description: Based on a review of five staff files, it was determined that one staff did not have complete documentation of orientation training. Evidence: Staff A, was hired at end of September or early October, the person who provided the orientation training did not sign the training document.

Plan of Correction: We will make sure the trainers sign all documentation of orientation training.
