An unannounced monitoring inspection was conducted today from 7:15am-8:45am. The school is closed due to the election and children are in the School Age Child Care program for the day. There were 34 children directly supervised by 2 staff. As children arrived to the center, they were accounted for by providing the staff their passport. The physical plant, 2 staff certifications, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in board games, reading books, playing with legos, having dramatic play and doing art projects. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-140-A Description: Based on record review, a child did not have documentation of a physical examination by or under the direction of a physician. Evidence: Child B does not have documentation of a physical examination in the child's record. Plan of Correction: We will contact parent and acquire a physical examination record.
Standard #: 22VAC40-185-60-A Description: Based on record review, two children are missing a work phone number and place of employment of each parent who has custody. Evidence: Child A and Child D are missing a work phone number for one of the parents. Plan of Correction: We will ask parents to complete the portion for the work phone number.
