An unannounced renewal was conducted from 10:10 a.m. - 1:30 p.m. There were thirty-four children in care with eight teachers working directly with them. There were two floaters available to assist as needed. Four children's records were reviewed. Two staff records were reviewed. The infant in care was observed sleeping. The older children were observed dancing, listening to a story and discussing the book being read, during hand washing, breakfast, lunch, music time and nap. Standard #: 22VAC40-185-140-A Description: Based on record review the facility failed to have prior to attendance or within one month after attendance documentation of a physical for one child in care. Evidence: 1. On 11/30/2016 the licensing inspector reviewed the record for child #1 and there was no documentation of a physical. The child came into care in August. 2. On 11/30/2016 staff #1 reviewed the record for child #1 and confirmed there was no documentation of a physical. Plan of Correction: I will notify the parent immediately and we should have this document for the record by Friday.
