Technical assistance was provided in the following areas of the standards: 22VAC40-185-(2)-160 (TB screening); 22VAC40-185-(7)-510 (Medication)

Comments:
An unannounced monitoring inspection was conducted on 12/5/16 from 3:25pm - 4:45pm. At the time of entrance there were 63 school age children in care with eight staff. Children were observed completing homework, participating in various activities, eating snack and playing outside. Records were reviewed for four children seven staff. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on a review of seven staff records, it was determined that the facility did not ensure that each staff member has documentation of a negative tuberculosis screening within 21 days after beginning employment and shall have been completed within 12 months prior to or 21 days after employment. Evidence: 1. The record for staff #3 (date of hire 8/26/16) contained documentation of a negative tuberculosis screening that was dated 10/3/16. 2. The record for staff #4 (date of hire 8/29/16) contained documentation of a negative tuberculosis screening that was dated 11/7/16. 3. The record for staff #6 (date of hire 9/8/16) contained documentation of a negative tuberculosis screening that was dated 10/26/16. 4. The record for staff #7 (date of hire 8/29/16) contained documentation of a negative tuberculosis screening that was dated 10/24/16. 5. Staff #8 (Program Director) confirmed that staff #3, #4, #6, and #7 did not have documentation of a negative tuberculosis screening within 21 days after employment.

Plan of Correction: All new staff will complete a TB screening within 21 days of employment.

Standard #: 22VAC40-185-60-A
Description: Based on a review of four children's records, it was determined that the facility did not ensure that they maintain and keep at the center a complete record for each child enrolled that contains all required information. Evidence: 1. The record for child #1 did not include the work phone number and place of employment for each parent. 2. Staff #8 (Program Director) confirmed the records for child #1 was not complete.

Plan of Correction: The parents of child #1 will be contacted to provide all missing information from their child's records. All new children's records will be reviewed prior to enrollment.
