Standard #: 22VAC40-185-160-C Description: Based on review of staff records, the facility did not obtain documentation of a negative tuberculosis screening at least every two years from the date of the initial screening. Evidence: Staff #1 had a tuberculosis screening dated 07/19/2014, exceeding the two year allowance to renew. Plan of Correction: Per the Director: I will send the staff today to obtain a tuberculosis screening.
Standard #: 22VAC40-185-350-C Description: Based on observation and interview, the facility did not maintain the staff to children ratio applicable to the youngest child in an ongoing mixed age grouping. Evidence: 1. There were 29 lower elementary children, aged 6-8 years with two staff in the main classroom. An additional child who appeared to be younger than the lower elementary children was observed on a nap mat in the main classroom. When questioned about the child on the mat, the Director stated that the child was three years old and from the primary classroom. The three year old changed the required ratio to one staff for every ten children. Plan of Correction: The Director took the three year old back to the primary classroom. Per the Director: We will do additional training regarding nap time ratios.
