Technical assistance was provided in the following areas of the standards: 22VAC40-185-(4)-270 (Building maintenance);

Comments:
An unannounced monitoring inspection was conducted on 12/7/16 from 11:15am - 12:45pm. At the time of entrance there were 22 children ages three years old to five years old in care with five staff. Children were observed participating in free play activities, eating lunch and during rest period. Records were reviewed for three children and two staff. There was no medication at the facility. Emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-290-3
Description: Based on observation, it was determined that the facility did not ensure that all unused electrical outlets shall have protective covers that are of a size that cannot be swallowed by children. Evidence: 1. There were two electrical outlets in the Preschool classroom that did not have a protective covering. 2. Staff #1 (Program Director) confirmed the protective covering was missing from both outlets.

Plan of Correction: Protective coverings were placed in the unused outlets during the inspection. All teachers will be re-trained to make sure all unused outlets have protective covers.

Standard #: 22VAC40-185-550-E
Description: Based on a review of the emergency drill log, it was determined that the facility did not ensure that it maintained a record of the dates of the practice drills for one year. Evidence: 1. There was no written documentation available for viewing during the inspection to demonstrate that the facility had completed the required monthly practice emergency evacuation drill for the month of November 2016. 2. Staff #1 (Program Director) stated they had been completed, but she forgot to document it.

Plan of Correction: The information about the emergency evacuation drill has been recorded on the emergency drill log. The emergency evacuation drill was completed on 11/23/16, but was not recorded on the emergency drill log. All future practice drills will be recorded by the Program Director after the drill is complete to ensure this won't happen again.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of two staff records, it was determined that the facility did not deny continued employment of a staff who did not have a search of the central registry finding within 30 days of employment. Evidence: 1. The record for staff #2 (date of hire 11/1/16) did not have documentation of a completed search of the central registry finding. Staff #2 was present during inspection and working with children. 2. Staff #1 (Program Director) confirmed that she had not received a completed search of the central registry finding for staff #2.

Plan of Correction: We will obtain the status of the background for staff #1. if it has not been received, we will complete anew form. For all new employees, we will ensure that we contact the appropriate office if the background check is not received within 30 days.
