Technical Assistance:

Comments:
An unannounced monitoring inspection was conducted on 12/07/2016 from 10:45am - 2:30pm. A tour of the facility and playground was conducted. There were 43 preschool children with five staff present. Seven staff records, three children's records and six medication reviews were conducted. Children were observed during morning program time and outdoor play. The violations are identified on the violation notice and were discussed with the director during the exit interview.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review and interview, the center failed to ensure that staff records contain documentation that two or more references as to character and reputation as well as competency were checked before employment. Evidence: 1. The record for staff #1 lacks documentation of a second reference. Staff #1 has a hire date of 8/24/2016. 2. Administrative staff confirmed that a second reference was not on file for staff #1.

Plan of Correction: Two additional reference letters have been obtained for staff #1 and placed in employee file.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a safe condition. Evidence: 1. The shed on the playground has widespread areas of deteriorated wood and peeling paint. 2. The cord to the AC window unit was dangling and accessible to children in the art room. 3. One window mini blind cord, in the art room, was dangling and accessible.

Plan of Correction: 1. The shed on the playground was evaluated by a craftsman and he will explore options to replace or repair worn areas of the shed. The areas will be covered in the next few months and there is a plan to do major repairs during the summer 2017. Inspection completed 12/9/2016. 2. The cord to the AC window unit was coiled and secured on 12/8/2016. Photo of correction sent to inspector. 3. The cord to the window mini blind has been secured on a hook out of reach of children on 12/8/2016. Photo of correction sent to inspector.

Standard #: 22VAC40-185-450-A
Description: Based on observation, the center failed to ensure that cots and mats used by children during the designated rest period shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges. Evidence: Two children were observed napping on rest mats without a bottom cover.

Plan of Correction: Parents have supplied additional covers for rest mats. Several more (4) sheets and blankets have been purchased by the school to keep in reserve if a child forgets one or for unexpected emergencies.
