Description: Based on review of written attendance records and staff interviews, the center did not maintain the required supervision ratio of staff to children. Evidence: One staff member were supervising eight children in the two's classroom. The required ratio of one staff member for every five children, based on the youngest child in the group, Child A (date of birth: 12/8/14; age 23 months) applies to the entire group.

Plan of Correction: The center will maintain the staff-to-child ratio based on the youngest child in a mixed group. The center will also obtain parent's written permission and a written assessment by the program director and program leader before children are assigned to a different age group.

Description: Based on staff interviews and a review of documentation, the center did not have parent's written permission and a written assessment by the program director and program leader for children assigned to a different age group. Evidence: Child A (date of birth: 12/8/14; age 23 months) was placed in the two year old classroom without written permission and assessment. Child B (date of birth: 1/6/15; age 23 months) were placed in the two year old class room without a written assessment.

Plan of Correction: The center will obtain written permission from the parent and complete a written assessment in order for child A to continue to be placed in a different age group. The center will obtain will complete a written assessment in order for child B to continue to be placed in a different age group. Moving forward the center will ensure that written permission and written assessments are completed before children are placed in a different age groups.

Description: Based on staff interviews, the center was allowing children to use equipment and materials that were not appropriate for their age group. Evidence: Children under the age of four were placed in the school age classroom from 6 AM until the teacher arrived to take them into the toddler classroom. There were toys and games with small pieces that created a potential choking hazard in the school age classroom.

Plan of Correction: The toddlers will no longer be placed in the school age classroom. Children ages two to four years old will be placed in the toddler room.
