Technical Assistance:
1. Discussed movement of children and maintaining ratio at start of shift. 2. Discussed moving children to new program, outside of age range, requiring assessment and documentation of parent approval.

Comments:
An unannounced monitoring inspection was conducted today from 8:00am-12:00pm. At the start of the visit, there were 32 children directly supervised by 5 staff. The physical plant, 3 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play and eating lunch. Diapering and hand-washing procedures were also observed. There was a sufficient supply of materials for the children. Areas of non-compliance are identified in the Violation Notice. Description: Based on record review, 3 of 5 children's records were incomplete. Evidence: 1. Child #1 was missing the parent's work place and number and an address for one emergency contact. 2. Child #2 was missing on parent's work number, both addressed for their emergency contacts, and documentation of acknowledgement communicable disease policy. 3. Child #3 was missing one parent's work place and number and the other parent's work number.

Description: Based on record review, 1 of 4 program leaders did not have all of the required qualifications. Evidence: 1. Staff 1 had no documentation to demonstrate experience or education to qualify them as a program leader (teacher).
