An unannounced monitoring inspection was conducted on January 6, 2017. Upon arrival to the facility, one staff was present with nine children. Throughout the course of the inspection there was two staff present with 24 school age children. Children were observed playing a variety of tabletop activities to include air hockey, board games, Legos, and talking freely amongst themselves. Three children's records were reviewed on this date. Licensing Inspector reviewed supervision, indoor and outdoor areas, activities, equipment, nutrition, injury report documentation, children's records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, liability insurance coverage, emergency supplies and required postings. There were no medications present in the center on this date. Exit interview conducted with the Recreation Supervisor (Staff #1).
Standard #: 22VAC40-185-60-A Description: Based on record review, in one of three children's records reviewed, the licensee did not ensure that a separate record for each child contained all required information. Evidence: During the inspection conducted on January 6, 2017, the record for Child #3 did not include documentation of complete addresses of two persons to contact in the event of an emergency where the parents cannot be reached. Plan of Correction: Complete addresses will be obtained from the parent at pick up.
Standard #: 22VAC40-185-340-C Description: Based on observation and inspection of the facility, the licensee did not ensure that during the stated hours of operation, there always shall be on the premises and on field trips when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them. The volunteer or other employee shall have received instruction in how to contact appropriate authorities if there is an emergency. Evidence: During the inspection conducted on January 6, 2017, upon arrival to the facility at 7:12 am , there was one staff member (Staff #1) with nine children. Staff #2 arrived at 7:24 am. Plan of Correction: Corrected during the inspection.
