22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection began January 3, 2017 and was completed on January 6, 2017. On January 3, 2017, staff records, children's records, medications and required postings were inspected. The licensing inspector returned on January 6, 2017 to complete the inspection. The following classroom child to staff ratios were observed:
Nine infants with three staff. The staff were feeding infants, and having floor play with infants.
Seven toddlers with two staff.
Fourteen two year old children with two staff. The children were creating water color paintings.
Sixteen three year old children with two staff. The children were building with blocks.
Eleven children Kindergarten through 2nd grade with one staff eating snack.
Nine older school age children with one staff eating snack.
Seventeen Pre-K children with two staff. The children were listening to a story and later had a short outside play time.
The last practice evacuation drill was conducted 12/29/16. Shelter-in-place drills for the year were conducted 2/24/16 and 9/21/16.
Three staff records and five children's records were inspected.

Standard #: 22VAC40-185-160-A
Description: Based on inspection of staff records, the center did not ensure staff submitted documentation of a negative tuberculosis (TB) screening within 12 months prior to or 21 days after employment. Evidence: Out of three staff records inspected, Staff #3 was re-hired 8/8/16. The documentation reflects a TB completed 3/25/14, which is over 12 months prior to hire.

Plan of Correction: Per Center: "A new screening will be conducted."

Standard #: 22VAC40-185-270-A
Description: Based on inspection of the facility, the center did not ensure all areas and equipment of the center were maintained in a safe and operable condition.
In the older school age classroom the following items were observed:
1. There is a hole in the wall on the left side of the classroom that measures approximately 2 inches.
2. The carpet in the back corner slips posing a tripping hazard.
3. The cabinet in the book area has a broken front door with exposed nails.

Plan of Correction: Per Center: "Painters were scheduled to come prior to the visit by licensing. We are scheduled for service this month. The area will be secure until repairs are completed. Rug removed."

Description: Based on inspection of the facility, the center did not ensure all hazardous substances such as cleaning materials were kept in a locked place.
In the toddler classroom there was a can of room freshener and a disinfectant that were unlocked.
In the two year old classroom, the closet where cleaners are stored was unlocked.

Plan of Correction: Per Center: "Additional training will ensure staff securely lock the area where cleaners are stored."

Standard #: 22VAC40-185-290-3
Description: Based on inspection of the facility, the center did not ensure all electrical outlets had protective covers. Evidence: In the three year old classroom, there were two electrical outlets that did not have protective covers.

Plan of Correction: Per Center: "Staff will ensure protectors are placed in plugs when not in use."
