An unannounced focused monitoring inspection was completed on 1/11/17 as a part of the probationary status of the center. There were 41 children present (including school-agers because schools were closed due to snow). No records were reviewed (no newly hired staff). Two medications were observed. The inspector arrived at the center at 11:30 am and departed at 2:00 pm. The children were observed finishing lunch, during nap, and during departure for a trip to skate for the school-aged children. There was discussion with the director about the following: completion of the bus emergency plans to include routes (and alternate) back to the center from sites regularly visited.
Standard #: 22VAC40-185-320-B Description: Based on observation and measurement, the center failed to ensure that the water in the rest rooms used by the children was maintained less than 120 degrees Farenheit. Evidence: 1. The hot water in the rest rooms used by the children was measured with a digital thermometer and the water was 123.5 degrees Farenheit. Plan of Correction: The maintenance person turned the water heater temperature down during the inspection. The temperature will be checked tomorrow and once a week for the next month to ensure the temperature is below 120 degrees.
Standard #: 22VAC40-185-510-A Description: Based on observation and interview, the center failed to ensure that medication for a child at the center had written authorization from the parent to be given to the child. Evidence: 1. There was a liquid pain relief observed with the medications. The medication had the name of a child in the center on the medication. There was no parent authorization for this medication. The manufacturer's box for the medication had not been opened and there was no label on the medication as to time/times to be given or dosage. The medication had not been given to the child. The director stated that the medication was found in the child's bookbag to be given back to the parent. Plan of Correction: The medication will be sent home today. In the future, medications that are not to be given to children will be marked as such.
Standard #: 22VAC40-185-560-F Description: Based on observation, the center failed to ensure that the menu of food provided by the center was posted. Evidence: 1. The breakfast menu of food provided by the center was not posted. Plan of Correction: This was corrected during the inspection. The front desk staff will ensure all three menus are posted monthly.
