We discussed the requirements for children's records and staff TB statements. The risk assessments for violations were provided. We talked about bathroom hand sink soap dispensers and use.
Thank you for your assistance during today's unannounced monitoring inspection conducted from 2:30 PM to 6:00 PM. Today, there were twenty-eight children in attendance after school with two to four staff. I viewed program activities, equipment, daily routines, staffing, supervision, interactions with children, posted information, snack, four records for children, four staff records, staff qualifications, staff training, agency inspection reports, emergency supplies and drill records, indoor areas. The children enjoyed active play and games in the gym. Hand washing routines were observed and staff provided reminders to individual children. Homework assistance was available in the cafeteria. Staff have provided table activities and today they made "winter books". Let me know if you have any questions. 540/430-9259
Standard #: 22VAC40-185-160-C Description: Based on a review of four records for staff, and an interview with administrative staff, the updated TB screening and statement for staff member 2 was not obtained before two years from the date of the last statement. Evidence: The statement from April 1, 2014 was not updated until April 25, 2016. Plan of Correction: Administrative staff are using a reminder system and will arrange for individual staff to obtain the updated TB screenings and statements in advance of the due dates.
Standard #: 22VAC40-185-60-A Description: Based on a review of four records for staff, and an interview with administrative staff, there was required information missing from the record of child 4. Evidence: There was a name listed by the parent as not authorized to pick-up child 4 and no accompanying legal paperwork. Administrative staff indicated that they had not inquired about any applicable legal paperwork at the time of enrollment in August 2016. During pick-up this date, the parent was asked and indicated she has legal paperwork to share. Plan of Correction: The site director will follow-up with the parent and obtain a copy of the legal paperwork, read over the applicable information, and keep the information in the file. The administrative staff will review all records to make sure records contain applicable legal paperwork if there are persons listed as "not authorized to pick-up". In the future, administrative staff will make sure all required information has been obtained when new children enroll.
