Comments:
An unannounced monitoring inspection was conducted from 9:00 am-12:00. At the time of entrance, 76 children were in care with 24 staff members present. Children were observed during snack time, engaged in group activities; play dough, legos, sensory table, diapering and hand washing. A selection of staff and children records, medications and other documents were reviewed. The violations were reviewed with the center director. Standard #: 22VAC40-185-70-A
Description: Based on record review, licensee has not maintained documentation of 2 or more references for each staff. Staff #1, hired 10/15/16 does not have documentation of a second reference on file. Staff # 2, hired 9/6/16 does not have documentation of a second reference on file.

Plan of Correction: I will get them this week.

Standard #: 22VAC40-185-520-C
Description: Based on observation and staff interview, staff have not obtained written parent authorization to apply diaper cream. Evidence: There is no written authorization from the parents for the staff to apply the diaper cream that was found in the child's basket.

Plan of Correction: We will ask the parent to complete a diaper cream form or send the cream home.
