An unannounced monitoring inspection was conducted on January 30, 2017 from approximately 3:20 pm to 5:10 pm. Two staff were present with twenty-five children. Additional staff were on site for administrative duties. Children were observed during dance and free play. Licensing Inspector reviewed standards related to supervision, activities, equipment, supplies, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, liability insurance coverage documentation, children & staff records, emergency supplies and required postings.
Standard #: 22VAC40-185-150-B Description: Based on review, facility did not ensure 1 of 2 children's health reports are signed and dated by the physician or designee. Evidence: On January 30, 2017, the record for child #2 has a physical and immunization record and neither are signed and dated by the physician or designee. Plan of Correction: Will ask parent when child is picked up today.
Standard #: 22VAC40-185-160-A Description: Based on review, facility did not ensure 1 of 2 staff have a TB test/screening within 21 days of beginning employment. Evidence: On January 30, 2017, the record for staff #1 (start date September 6, 2016) did not have a TB test or screening. Plan of Correction: Staff will get the results from her physician.
Standard #: 22VAC40-185-280-B Description: Based on observation, facility did not ensure hazardous substances are kept locked. Evidence: On January 30, 2017, peroxide cleaner spray labeled "caution...keep out of reach of children" was observed sitting on the paper towel dispenser in the dance room where children were observed playing. Plan of Correction: Will put it in the janitor's closet.
Standard #: 22VAC40-185-510-J Description: Based on observation, facility did not ensure medication is kept locked. Evidence: On January 30, 2017, Immodium caplets were observed in a first aid kit on a shelf in a office that was not locked. Plan of Correction: Will move that whole shelf back into the staff closet.
Standard #: 22VAC40-185-540-B Description: Based on observation, facility did not ensure first aid kit is not accessible to children. Evidence: On January 30, 2017, three first aid kits were stored on a low cabinet just inside an open office that is accessible to children. Plan of Correction: Will move that whole shelf back into the staff closet.
