Technical Assistance:

The licensing inspector conducted an unannounced monitoring inspection on Thursday, January 19, 2017 from 11:54 am to 1:40 pm to determine the center's compliance with licensing standards. During this inspection, the Director and the owner of the center were present and accompanied and assisted the licensing inspector throughout this inspection. The census for today's inspection consisted of 13 children and three staff. The licensing inspector completed observation of the children on the first and second floor (see violation) levels. The children were observed on the second level of the facility eating lunch and preparing to transition to the lower level for nap time. The children's lunch consisted of ravioli, corn, pears, and milk. Interviews were held with staff throughout the inspection, and the inspector interacted with children in the second level classroom when appropriate. The children on the first floor were observed by the licensing inspector napping in the designated areas. The classrooms were equipped with age appropriate toys, and supplies. The staff were observed providing adequate supervision and addressing the children's overall needs. The center provides care to school aged children and they were not present during this inspection. There are multiple staff with current CPR, First Aid and Daily Health Observation training on site. The center has one staff MAT certified at the facility. During this inspection, the required postings were reviewed and determined to be in compliance. The following information was reviewed: daily attendance, lunch menu, license, most recent inspection summary, injury reports, fire and shelter-in-place drills, liability insurance policy, and emergency preparedness plan. There were four children?s records and three employee records reviewed during this inspection. Medication is being administered and was reviewed.
The center's first aid kit and emergency supplies were inspected.

Standard #: 22VAC40-185-40-E
Description: Based on observation of the facility, the center did not ensure that the center's activities, services, and facilities comply with own policies and procedures that are required by these standards and the terms of the current license.

Evidence: During this inspection, the licensing inspector completed observation of the children and staff utilizing the second level of the facility (upstairs area) which is not in compliance with the current license issued by the department.

Plan of Correction: Per the center: We are currently using the first floor only. Follow through with the process to modify current license.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on a review of staff records, the center did not ensure the most recent criminal background checks were updated before three years since the last date of the sworn statement. Evidence: In Staff#1's record an updated sworn statement was not found. The sworn statement reviewed in the record was last dated 05/31/13. The background checks are required to be updated every three years.

Plan of Correction: Per the center: Staff will sign document today and a copy will be placed in the file.
