63.2 Child Abuse and Neglect
- Technical Assistance:
Discussed the camera system and the problem with the footage being deleted every 7 days even after a parent request to view the footage.
An investigation of a center reported concern was investigated at the above dates. A violation notice was presented to the center and the ongoing compliance with the licensing standards is the responsibility of the licensee. Standard #: 22VAC40-185-40-E Description: The center failed to use the face to name sheets on Dec 22nd when a group of children went to the playground. Evidence: Based on the review of records the center staff failed to follow the guidelines for the face to name sheets. The center staff did not record when children went to and from the playground and when staff left. Plan of Correction: We have trained all staff at an emergency staff meeting on January 3rd. At that staff meeting we covered, face to name , attendance tracking procedures, schedules and use of walkie talkies and weekly sign in and out sheets.
Standard #: 22VAC40-185-340-F Description: A child was left out of sight and sound supervision in the preschool room for an undetermined amount of time. Evidence: A group of 13 children were on the playground at 4:30 in the afternoon on the 22nd of December. At one point a staff went inside with three children to wash hands, 5 minutes later when she came back she could not find a three year old child. After walking the perimeter of the playground the teacher observed the child coming out of the door from the room. The staff recalls seeing the child on the playground when they first went outside and can not recall the child going inside. Plan of Correction: We have discussed this at the emergency staff meeting and staff involved took the companies safety and supervision training. Admin staff are spot checking face to name sheets to make sure they are done.
Standard #: 63.2-1509-A Description: The center failed to report the incident of a child being left in a room at the center to the local CPS. Evidence : On December 22nd the center had a staff report that a child was missing from the playground and observed coming out of a room that did not have any teacher in it. The center reported the incident to licensing on the 27th of December. But failed to report to the local CPS. Plan of Correction: We will make sure we report to cps.
