Technical Assistance:
Discussed playground plan and supervision for the new set up.

Comments:
An unannounced renewal was conducted from 10 - 12. The Inspector walked around the school, observed classes, interviewed staff, sat with preschool children, observed toddlers and infants, and observed special feeding procedures in the infant room. The Inspector reviewed records, emergency drills, menu, daily schedules, and infant reports. Ratios were followed, supervision was noted, and age appropriate materials were available. Standard #: 22VAC40-185-40-J
Description: Based on observation and staff interviews, the annual Injury Prevention Plan is not complete as it does not include plans of staff and implementation. Records by staff have been reviewed, tallied, and summaries are complete but the implementation on what steps to prevent are not completed.

Plan of Correction: We are finishing the plan and will train staff in the next meeting.

Standard #: 22VAC40-185-270-A
Description: Based on observation of outside and inside, the following items are not maintained in a clean manner: 1. Toddler room's floor has sticky tape residue and dirt and dust around little fridge. It also has wall repairs that expose sharp edge. 2. Pre K room's wooden door has splintered by the door handle. There is also a broken outlet by the sink. 3. Preschool class has wooden chairs that are dirty. 4. Two's class has floor damage from bathroom door and one set of blinds are wrapped in a way that does not allow use. 5. One playground has mud by structure and a cloth towel and window screen were observed on the ground.

Plan of Correction: We are in the middle of renovations and will contact the crew to ensure they clean up and leave facility in proper manner. We have ordered new cleaning product to address wood furniture because staff have been cleaning furniture. We will put in work orders for door and walls. Our playground has been approved and remodeling begins in the next 6 weeks.

Standard #: 22VAC40-185-280-B
Description: Based on observation of school, the center staff are not removing keys out of doors that contain cleaning items behind those doors.

Plan of Correction: We will remind staff to take out the keys and put them away.

Standard #: 22VAC40-185-320-B
Description: Based on observation, the center staff are not ensuring toilet paper is kept by each toilet. The boys bathroom between the Preschool room was not equipped with toilet paper and the two's bathroom had toilet paper stored where children could not reach.

Plan of Correction: We will remind staff to check bathrooms and equip when necessary.
