An exit interview was conducted with the director to discuss the results of today's inspection. Consultation was given regarding the subsidy inspection process and subsidy agreement.
An unannounced monitoring inspection was conducted today, January 17, 2017 to determine the center's compliance with licensing standards. The inspection was initiated at 3:30pm and concluded at 5:10pm. Upon arrival, the following ratios were observed:
One-two year olds room: 1 staff/ 4 children
Two year olds room: 1 staff/ 5 children
Two and a half to three year olds room: 1 staff/ 5 children
School age room: 2 staff/ 8 children
Children were observed interacting with staff, coloring pictures, playing with toys, and participating in organized activities in the gymnasium. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. All classrooms were inspected to include main hallways and children?s restrooms. Due to inclement weather the outdoor play area was not inspected.
The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, food menu, previous inspection, and various parent information. These postings are located on the wall upon entering the center and in children's classrooms. The center's first aid kit and emergency supplies were inspected with additional supplies found. Five children's records were inspected with the majority of the required information found captured; see attached notice. Staff records were accessible for inspection. The following was also inspected: fire/health inspections, fire/shelter-in-place drills, parent agreements, injury prevention plan, parent agreements, and other pertinent information.

Standard #: 22VAC40-185-60-A Description: Based on the inspection of children's records the required information was not present.
During the inspection of five children's records, child #1 and #2 did not have the physician's name and telephone listed in the record.
Plan of Correction: Per director: I will get this information from the parent during pickup time.
