Comments:
The licensing inspector conducted an unannounced monitoring inspection from 10:10am - 12:15pm. There were sixteen preschool children in care with three staff and one volunteer. The children were observed arriving at the center, finishing breakfast, during hand washing and playing in the centers. Two staff's and three children's records were reviewed. Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to have hazardous substances stored in a locked place. Evidence: 1. The inspector observed the classroom. The cleaning supplies including toilet bowl cleaner, Clorox Anywhere, Pine Sol and Lysol cleaner were stored in a wall cabinet. The cleaning supplies were inaccessible to the children, but there was no lock or safety latch on the cabinet.

Plan of Correction: The teacher said the safety latch broke and a new one has been requested from the central office. The teacher will call the central office today to check to see if a safety latch has been purchased for the cabinet.

Description: Based on record reviews, the center failed to include all required information on an injury report. Evidence: 1. The inspector reviewed an injury report. There was only one signature on the injury report and any future action to prevent recurrence of the injury was not included on the report.

Plan of Correction: The teacher said in the future she would make sure there are two signatures on the injury reports and actions to prevent recurrence of the injury will also be included on the reports.
