Comments:
An unannounced inspection was conducted from 12:15pm to 2:10pm. At the time of entrance, there were 29 children present with 6 staff present and 1 administrator. The inspector observed children participating in nap/ quiet time, free play and infant activities, toileting and hand washing, diapering procedures. A sample size of 5 children's records and 2 staff records were reviewed, in addition to medication and injury reports. Areas of noncompliance are identified in the Violation Notice and were discussed with the Director at the conclusion of this inspection.

Standard #: 22VAC40-185-70-A
Description: Based on record review it was determined that the center did nor ensure that each staff record contained name, address and telephone number of a person to be notified in an emergency which shall be kept at the center. Evidence: The record for staff #1 did not contain the address of a person to be notified in an emergency.

Plan of Correction: Director will have staff #1 provide this information for the record.

Standard #: 22VAC40-185-290-3
Description: Based on observation it was determined the center did not ensure all unused electrical outlets had protective covers. Evidence: In the infant room their was an uncapped wall outlet in the main room.

Plan of Correction: Corrected during this inspection.
