Comments:
Thank you for your time and assistance with the unannounced renewal inspection conducted today, February 21, 2017. The on-site inspection was from 8:45a.m. to 11:50a.m. On this date, there were 16 children present and in the care of two staff members. There were a total of 39 children enrolled in the two morning classes of the preschool program. There were three children's records reviewed and three staff records reviewed. There were two staff members interviewed, as well as four children. There were no medications being administered on this date which is the policy of the center. Snack was provided by the center and consisted of cheese balls and apple juice. The exception to compliance was noted on the violation notice and the risk rating was noted on the supplemental page. The exit interview was conducted on this date. The children enjoyed learning the new song, "Cover When You Cough" and working on a color sorting activity with Fruit Loops. Some items observed and reviewed included: posted monthly snack menu, posted daily schedule, posted emergency evacuation map, posted license and current inspection findings, arrival procedures, choice time, opening time, transitions, morning snack, hand washing and bathroom procedures, outdoor time, circle time, story time, and dismissal procedures. Standard #: 22VAC40-185-40-G
Description: Based on review of three children's records and verification from the staff, the center failed to obtain proof of age and identity for one child. Evidence: 1. The record for Child #2 did not contain documentation that the parent had provided proof of the child's identity and age. Date of enrollment for the child was 02/02/2017. 2. Staff Member B verified that they had not received proof of the child's birth from the parent.

Plan of Correction: The staff will make contact with the parent of the child today and ensure that proof of birth is obtained. In the future, if the proof of identity and age has not been received within seven days of enrollment, the staff will contact the local law enforcement.
