The following Monitoring Inspection took place between the hours of 12:30pm and 2:40pm Today, I met with the staff of the Creative Play School I, located in Alexandria, Virginia. At the start of this unannounced monitoring inspection there were a total of 66 children with 10 staff; good ratios in all 5 classrooms. A total of 7 children?s files and 5 staff files were reviewed. Corrective actions verified from the last licensing inspection in October 2016. The children were engaged in the following: handwashing and preparing for lunch which consisted of pasta, salad and milk, nice transition observed from lunch to naptime. Six medications currently all were reviewed along with medication authorizations. Injury log, evacuation drills and shelter in place drills were also reviewed. Standard #: 22VAC40-185-270-A Description: Based on observation of physical plant, areas of the building were found not maintained in a clean, safe and operable condition. Evidence: In the two year old classroom the air conditioner has dust layers in the vents. In the toddler room, two cots were observed with torn sheets at the corners. In the preschool 2 classroom the pipe covering from the radiator on the bottom right side is torn and coming apart. In the preschool 2 classroom the cinder block wall has a gap between the cinder blocks at the bottom of the wall which is accessible to children and poses a hazard. Plan of Correction: All areas of the building that were out of compliance were corrected during the inspection.
Standard #: 22VAC40-185-280-B Description: Based on observation of the physical plant, an area of the building had hazardous substances that were accessible to children in care. Evidence: The power room in the main hallway was found unlocked and had cleaning supplies and salt for icing. Plan of Correction: Director locked the power room door during the inspection.
