Description: Based on observation, a closet in the multi-purpose room was not maintained in a operable condition. Evidence: The wall board has a large hole and visible pieces of wall board are laying on the floor.

Plan of Correction: The Center Director will request the facility maintenance to add a lock to the door to keep it inaccessible to the children.

Description: Based on observation, a custodial closet was unlocked and the door ajar when the Inspector walked through the bathrooms. Evidence: The closet door was left open and cleaning bottles were accessible.
