Comments:
20 Children were in care during the inspection, The age range of the children was 10 months to five years old. Children were observed during lunch , free play and rest. No children were on medications and three staff are current in MAT. Five staff files were reviewed. Inspection time 12:15pm- 2:30pm.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review of six children's files not all were complete. Evidence: Child A 's file was missing the street address for one emergency contact.

Plan of Correction: We will ask the parents for the missing information.

Standard #: 22VAC40-185-500-B
Description: Based on observation not all diapering surfaces were cleaned . Evidence: The staff did not clean the diaper pad with soap and water before disaffecting.

Plan of Correction: We will remind on diapering procedures.

Standard #: 22VAC40-185-570-E
Description: Based on observation not all bottles were labeled. Evidence: The infant's bottles were not labeled with the child's name or dated.

Plan of Correction: We will remind staff to label all bottles.
