The licensing inspector for Childcare Network conducted an unannounced inspection in response to a self reported issue that was received by the licensing office on 2/22/17. Interviews were conducted with staff relating to the allegation of lack of injury reporting. The preponderance of the evidence gathered during the investigation does support the allegation, so the incident is determined to be "valid".
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and returned it to me within 5 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person responsible for implementing each step and/or monitoring any preventive measures.

Standard #: 22VAC40-185-280-B
Description: Based on inspection of the facility, the center did not ensure all hazardous substances such as cleaning materials, insecticides and pesticides were kept in a locked place. Evidence: A closet located in the hallway was not locked and contained insecticides and spray paints.

Plan of Correction: Per Center: "Cabinet was locked during visit and new lock will be put in place."

Standard #: 22VAC40-185-430-A
Description: Based on inspection of the facility, the center did not ensure outdoor play equipment used by the children was age appropriate. Evidence: A two year old child was injured while using a tricycle with a manufacturer age requirement usage for children ages three and four years old.

Plan of Correction: Per Center: "Instructed all two year old teachers that the tricycles are not to be used by two year olds."

Standard #: 22VAC40-185-500-A
Description: Based on observations, the center did not ensure children washed their hands after diapering/toileting. Staff did not wash their hands after assisting with diapering/toileting.
Three children were observed being changed on the diapering table. After each diapering, the child was permitted to return to play without washing their hands. The staff changing the diapers did not wash their hands between changing each child's soiled diaper.

Plan of Correction: Per Center: " Retrained staff on handwashing and diapering procedures."

Standard #: 22VAC40-185-500-B
Description: Based on observations, the center did not ensure there was an appropriate container for soiled diapers. Evidence: The staff changing soiled diapers disposed the diapers in an open container. The standards require soiled diapers to be disposed in a storage system that is foot operated.

Plan of Correction: Per Center: "New step on cans will be purchased for diaper changing classrooms."

Standard #: 22VAC40-185-550-L
Description: Based on interviews and review of documentation, the center did not ensure a parent was notified of their child's injury the day of the injury. Evidence: A parent did not receive an injury report documenting her child's injury that occurred on February 21,2017.

Plan of Correction: Per Center: " Retrained staff and had them sign off on policy and procedure."
