Comments:
An unannounced monitoring inspection was conducted at the facility March 7, 2017 from approximately 1:25 pm - 5:41pm. Upon entrance there were 131 children and 15 staff. Additional staff were on site for administrative support. The Licensing Inspector observed rest/nap time, outside play , coloring, letter and object recognition. Licensing inspector reviewed 4 children's records, 2 staff records, 5 board members records, 5 medications and medication documentations and 5 injury documents. Exit interview conducted with director.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on 1 of 2 staff records reviewed, the licensee did not obtain documentation of a negative tuberculosis screening with 21 days after employment. Evidence: During the inspection conducted on March 7, 2017, staff #1 (date of hire 8-14-15) did not have a documentation of a tuberculosis screening.

Plan of Correction: Form was given to staff to complete.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the licensee did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: During the inspection conducted on March 7, 2017, the licensing inspector observed the following on the Playground closest to the building the wooden border by the swing area is deteriorating and splintering by the walk way. On the School Age Playground area there was a bench with one board broken, splintering and with sharp edges.

Plan of Correction: The maintenance staff will replace boards.

Standard #: 22VAC40-185-330-B
Description: Based on observation and inspection of the playground, the licensee did not to ensure that the required amount of resilient surfacing was under playground equipment with moving parts or climbing apparatus to create a sufficient fall zone. Evidence: During the inspection conducted on March 7, 2017 , there was an inadequate amount of resilient surfacing in the fall zone on the following playground areas. 1. On the playground composite #1 the curvy slide depth of the pea gravel measured at 2 inches at the end of the chute of the slide. The 2nd slide on playground composite #1 at the end of the chute the depth of the pea gravel was approximately 4 1/2 inches. 2. The swing area closest to the building the pea gravel and sand were mixed and it was unable to be measured. 3. On the playground furthest from the building all of the slides depths of the pea gravel measures from 0 inches- 4 inches in the 1 to 3 feet fall zone area .

Plan of Correction: Maintenance staff will move the pea gravel to the fall zone area.

Standard #: 22VAC40-185-340-F
Description: Based on observation, the licensee did not ensure children under 10 years of age always were within actual sight and sound supervision of staff. Evidence: During the inspection conducted on March 7, 2017 at approximately 1:30pm licensing inspector observed staff #1 and staff #2 in classroom 114 talking to staff #3. Staff #1 stated she left the classroom to ask a question while the children were sleeping. Staff #1 confirmed there was no staff member in classroom 118 that was in actual sight and sound of the children when she was in classroom 114. In classroom 118 , there were 7 children unsupervised.

Plan of Correction: The staff was addressed that children will need to be in actual sight and sound at all times. The incident will be documented.

Standard #: 22VAC40-185-440-E
Description: Based on observation, licensee did not ensure there is at least 12" of space between occupied cots during nap time. Evidence: During inspection conducted on March 7, 2017, licensing Inspector observed in the following classroom cots that did not have 12 inches of space between occupied cots. In the classrooms for 2 year old - three year olds,, 9 of 23 cots did not have 12 inches of space between of occupied cots. In the classroom for 1 and 1/2 year olds - 2year old, 13 of 25 cots did not have 12 inches of space between of occupied cots.

Plan of Correction: Staff will be retrained on spacing cots 12 inches a part.
