Comments:
The licensing inspector conducted an unannounced monitoring inspection Thursday, March 09, 2017 from 10:45am to 12:40pm. The children were observed playing on the playground, listening to stories, singing songs and reading books. The infants were observed being fed, playing with toys, sleeping and having tummy time on the floor. Lunch consisted of macaroni and cheese, peas, pears and milk. The menu was posted. All classrooms and playgrounds were inspected today as well as the facility?s mini bus. Four children?s records and two employee records were reviewed during this inspection. The center administers medication; however they did not have any being given at this time. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 02/27/2017 Last shelter-in-place drill: 01/19/2017 Last fire inspection: 07/28/2016 Last health inspection: 02/23/2017 Total enrollment: 106 Today, the following child to staff ratios were observed: Infants ?10:3 Toddlers ?4:1 Twos ? 12:2 Preschool ?13:2 Pre-K ?9:1 School Age ?not present The violations from the previous inspections were checked for corrections. There was one repeat violation found during today?s inspection. If you have any questions about this inspection, please contact the licensing inspector, Roshon Casey, at (804) 662-9760. PROVIDER: Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today (03/16/2017). You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 22VAC40-185-160-C
Description: Based on a review of staff records on 03/09/2017, the center did not ensure that two of two staff records reviewed contained documentation a tuberculosis (TB) screening every two years. EVIDENCE: 1. The most recent documentation of a TB test in the record of Staff #1 is dated 10/23/2014. 2. The most recent documentation of a TB test in the record of Staff #2 is dated 12/12/2014.

Plan of Correction: Going Forward a monthly check on all files to ensure that no one is behind on TB tests. These two staff members are current as of 03/10/2017

Standard #: 22VAC40-185-270-A
Description: Based on observation on 03/09/2017, the center did not ensure that all areas of the center inside and outside were maintained in a clean, safe and operable condition. EVIDENCE: The seat on the second row on the left had a tear. The foam cushion was exposed on the seat.

Plan of Correction: Corrected with duck tape while awaiting to go into the shop.

Standard #: 22VAC40-185-520-A
Description: Based on observation on 03/09/2017, the center did not ensure that over-the-counter skin products shall not be kept or used beyond the expiration date of the product. EVIDENCE: 1. A tube of diaper cream belonging to Child #5 was observed in his cubbie with his diapers. The cream expired on 02/2017. 2. A tube of diaper cream belonging to Child #6 was observed in his cubbie with his diapers. The cream expired 04/2016. Staff #2 stated that the cream was used on the child "a couple of weeks ago".

Plan of Correction: Going forward all creams are checked for expired date before left with teacher.

Standard #: 22VAC40-185-520-C
Description: Based on observation and staff interview on 03/09/2017, the center did not ensure that when diaper ointment is used that the product was labeled with the child's name. EVIDENCE: An unlabeled tube of cream was observed in a cubbie with diapers. Staff #2 stated that the cream belonged to Child #6. Staff #2 also stated that the cream was used on the child "a couple of weeks ago".

Plan of Correction: Going forward all medications are labeled with child's name.
